Apicoectomy in Thousand Oaks

Apicoectomy in Thousand Oaks

When a root canal-treated tooth keeps causing trouble, a second root canal is not always the right answer. Sometimes the problem sits at the very tip of the root, where infection or inflammation lingers in the surrounding bone. In those cases, an apicoectomy offers a way to treat the source directly while keeping your natural tooth in place. At 805 Dentistry in Thousand Oaks, our board-certified endodontist performs this procedure in the same office where our patients already receive their family, cosmetic, and specialty dental care.

An apicoectomy, sometimes called root-end surgery, is a minor surgical procedure that treats the tip of a tooth's root, known as the apex. Instead of reaching the infection through the top of the tooth, as a root canal does, we access it through the gum tissue near the root. We then remove the inflamed or infected tissue along with the very end of the root, and we place a small filling to seal the root canal from that end.

This approach is useful because some root canal systems have tiny branches, curves, or hidden spaces that are difficult to clean from the crown of the tooth. If bacteria remain in those areas, discomfort or infection can return even after careful treatment. An apicoectomy lets us address that remaining problem area directly. For many patients in Thousand Oaks, the procedure can be an alternative to extraction and the tooth replacement that would follow.

Apicoectomy is usually considered for a tooth that has already had a root canal but continues to show signs of trouble. That trouble might be lingering tenderness, swelling near the root, or an infection visible on imaging. It may also be recommended when a crown or post makes retreatment through the top of the tooth impractical, or when the root's shape prevents complete cleaning.

Because our practice also performs root canal retreatments, we can compare both options for your specific tooth. Our office provides CBCT imaging and 2D and 3D dental imaging, and we have a board-certified oral and maxillofacial radiologist on staff. Detailed 3D views help us see the root, the surrounding bone, and nearby structures before we recommend a plan. Patients do not need a referral to see our endodontic team.

What happens during an apicoectomy?

The procedure is typically completed in a single visit using local anesthetic to numb the area. Before we begin, we review your imaging with you and explain exactly which tooth and root we are treating.

During treatment, here is what you can generally expect:

  • A small opening is made in the gum tissue near the affected root
  • Inflamed or infected tissue around the root tip is gently removed
  • The very end of the root is trimmed away
  • A small filling is placed to seal the end of the root canal
  • The gum tissue is repositioned and may be closed with a few small stitches

If dental anxiety makes surgery feel daunting, we can help. Our board-certified dental anesthesiologist offers nitrous oxide, oral sedation, IV sedation, and general anesthesia, so we can talk through which level of comfort support suits you.

What is recovery like after an apicoectomy?

Most patients can expect some mild swelling and soreness for a short period afterward, and we give you clear written aftercare instructions before you leave. These typically cover managing discomfort, eating soft foods, keeping the area clean without disturbing it, and avoiding chewing on the treated side at first.

We schedule a follow-up visit to check how the gum tissue is healing. Over time, imaging can show us how the bone around the root tip is responding. If you have questions or concerns during recovery, call our Thousand Oaks office. Because we offer same-day emergency appointments for endodontic concerns, you will not be left waiting if something does not feel right.

How can I pay for an apicoectomy?

We accept all major PPO insurance plans, and our team will help you understand your coverage before treatment begins. Endodontic services in our office are filed with insurance as specialist care, and we will explain how that applies to your plan.

We also welcome self-pay patients, and financing is available to help spread out the cost of care. At your consultation, we will go over the expected costs of your treatment so you can make an informed decision.
