An onlay rebuilds a damaged tooth by covering only the parts that need help. When decay or a fracture reaches one or more of the cusps (the raised points on a back tooth's chewing surface) but much of the healthy structure remains, an onlay can restore strength without reshaping the entire tooth the way a full crown does. At 805 Dentistry, Dr. Dar Radfar and our team place onlays for patients from Calabasas and nearby communities who want a durable repair that keeps as much natural tooth as possible.

An onlay is a custom restoration that fits into and over the chewing surface of a tooth, including one or more cusps. It sits between a filling and a crown. A filling repairs damage inside the tooth's grooves. An inlay fills the space between the cusps. An onlay extends over the cusps to protect them. A crown covers the entire visible tooth.

The difference matters because every millimeter of healthy enamel we keep helps the tooth over the long term. When a cavity or crack is too large for a filling but not extensive enough to require a crown, an onlay lets us remove only the damaged area and rebuild the rest. Our office offers fillings, inlays, onlays, and crowns, so we recommend the option that matches the actual condition of your tooth.

Many of our Calabasas patients consider an onlay for one of these reasons:

  • A large or old filling has broken down and no longer protects the tooth
  • A cusp has cracked or chipped off a molar or premolar
  • Decay has spread beyond what a standard filling can reliably support
  • A tooth is weakened but still has enough healthy structure to avoid a full crown

Not every damaged tooth is a good fit. If decay has reached the nerve, a root canal may be needed first. Our office includes an endodontics practice with a board-certified endodontist who treats cracked teeth and performs root canal therapy. If too little healthy tooth remains, a crown is usually the stronger choice. We make that call after a dental exam and imaging, so your treatment is based on what we find rather than a one-size-fits-all approach.

What happens during an onlay appointment?

Treatment usually takes two visits. At the first visit, we numb the area, remove the decay or weakened structure, and shape the tooth so the onlay will fit securely. We then take an impression and place a temporary restoration to protect the tooth.

At the second visit, we remove the temporary, check the fit and bite of your custom onlay, and bond it in place. We make small adjustments so it feels natural when you chew. Our office has an on-site dental lab, which supports close coordination on restorative work. If dental visits make you anxious, we offer nitrous oxide, oral sedation, and IV sedation, and a board-certified dental anesthesiologist is on our team.

How do I care for an onlay afterward?

Care for an onlay the same way you care for your natural teeth: brush twice a day, floss daily, and keep up with regular cleanings and exams. The edges where the onlay meets the tooth still need thorough cleaning, because decay can form there just as it can around any restoration.

After the temporary is placed, avoid sticky or very hard foods on that side until your final onlay is bonded. Once it is in place, let us know if your bite feels uneven or the tooth stays sensitive so we can check it. If you clench or grind at night, ask us about a night guard. We also treat TMJ disorders and evaluate jaw-related problems that can put extra stress on restored teeth.

How can Calabasas patients pay for an onlay?

We accept all major PPO insurance plans, welcome self-pay patients, and offer financing. Onlays are a restorative treatment, so many PPO plans provide some coverage. The amount depends on your specific plan, and we can review your benefits with you before treatment begins.

If you don't have insurance, or your plan leaves a balance, financing can spread the cost over time so you don't have to delay care for a tooth that is already compromised. Waiting can allow a crack or cavity to grow until a larger restoration is needed. We will walk you through the options at your consultation so you know what to expect before we begin.

Why do Calabasas families choose our office for restorative care?

Calabasas is one of several communities we serve, and many families appreciate having general, cosmetic, and specialty care under one roof. We see children and adults of all ages. Our team includes specialists in endodontics, periodontics, oral surgery, oral pathology, radiology, orofacial pain, and dental anesthesiology. If an onlay turns into a more involved case, you can often stay with the same practice instead of starting over somewhere new.

We also offer emergency dental appointments. That helps if a cracked cusp or broken filling catches you off guard. If you live in Calabasas and have a tooth that needs more than a filling, contact 805 Dentistry to schedule an exam and find out whether an onlay is the right fit.
