A molar with a cusp that cracked off around an old filling is a common reason patients come to us. The tooth is often too damaged for another filling but still has plenty of healthy structure worth saving. In many of these cases, an onlay is the right middle ground. Our office in Thousand Oaks serves patients throughout the Conejo Valley, and we regularly help Agoura Hills patients restore teeth like this with onlays crafted in our own on-site dental lab.

An onlay is a custom restoration that covers and rebuilds part of a tooth's chewing surface, including one or more of its cusps, the raised points on back teeth. It is closely related to an inlay, which fits within the grooves between the cusps. An onlay extends over a cusp to reinforce areas that have broken or weakened. Because it replaces only the damaged portion, an onlay is often described as a partial crown.

For many Agoura Hills patients, the appeal is conservation. A full crown requires reshaping the entire tooth so the crown can cover it. An onlay lets us keep more of your natural tooth while still restoring the parts that can no longer handle everyday chewing on their own. It is one of several restorative options we offer, alongside fillings, inlays, and crowns, so we can match the treatment to the tooth rather than the other way around.

Onlays can be a good fit for a range of situations. During your exam, we look at how much healthy tooth remains and where the damage sits. Common examples include:

  • A large, aging filling that is breaking down or has decay forming around its edges
  • A cusp that has chipped or fractured while the rest of the tooth stays sound
  • Decay that has spread too far for a filling to restore the tooth reliably
  • Wear on a back tooth that has weakened part of the biting surface

Patients often like that onlays are custom-made to fit their tooth and bite, and that they preserve natural tooth structure compared with a full crown.

Not every damaged tooth is a candidate. If a crack extends deep into the tooth or the nerve is involved, a different approach may serve you better. Because our practice includes a board-certified endodontist who treats cracked teeth and performs root canal therapy, we can evaluate these more complex cases without sending you elsewhere. If an onlay isn't the best choice, we will walk you through the alternatives and explain why.

What happens during an onlay appointment?

Getting an onlay typically takes two visits. First, we examine the tooth and take any imaging needed to understand the damage. At the first visit, we numb the area, remove decay or the old filling, and shape the tooth so the onlay fits securely. We then take an impression and place a temporary restoration to protect the tooth while your onlay is made.

Your onlay is fabricated in our on-site dental lab, which lets our team work closely on the fit and shape of your restoration. At your second visit, we remove the temporary, check the fit and your bite, and bond the onlay in place. If dental visits make you anxious, we offer sedation options, including nitrous oxide and oral sedation, to help you feel more at ease throughout treatment.

How do I care for my onlay afterward?

You care for an onlay much as you would your natural teeth. Brush twice a day, floss daily, and pay attention to the edges where the onlay meets your tooth, since keeping that area clean helps protect against new decay. While you have the temporary restoration, chew gently on that side and avoid sticky or very hard foods.

Regular cleanings and exams let us check your onlay and the surrounding tooth over time. If you grind or clench your teeth, mention it to us. We offer night mouth guards, which can help protect your restorations and natural teeth from that extra pressure. Our office also offers emergency dental appointments if something ever feels wrong.

How can I pay for my onlay?

We accept all major PPO insurance plans, and our team will help you understand how your coverage applies to your onlay before treatment begins. Coverage varies from plan to plan, so we review your benefits with you up front so you know what to expect.

We also welcome self-pay patients, and financing is available for those who prefer to spread out the cost of care. Families in Agoura Hills choose us as a one-stop shop for general, cosmetic, and restorative care, led by Dr. Dar Radfar, DDS. If you have a tooth that's chipped, worn, or carrying an old filling that's seen better days, contact our office to schedule an exam and find out whether an onlay is right for you.
